SNS covers psychology and psychiatry, but waiting lists are 3–9 months for psicología clínica in most CCAAs. Crisis lines are free and 24/7 (024 national suicide prevention since 2022). English-speaking therapists exist in Madrid, Barcelona, Valencia, Málaga, the Balearics and Canaries but are scarce in public system — private therapy €50–90/session is the common expat route.

FreeAccessing Therapy
Through public insurance: Start with your médico de familia at the Centro de Salud. They refer you to Salud Mental — a community mental health unit in your area. First appointment with psiquiatra or psicólogo clínico typically within 2–6 weeks for initial screening. Subsequent therapy sessions: 4–8 weeks apart once established. SNS therapy is free.
Waiting times: 3–9 months for regular psicología clínica in big cities (Madrid, Barcelona, Valencia). Urgent cases (post-crisis, suicide attempt follow-up) expedited within 1–2 weeks. Rural CCAAs often quicker (1–3 months) but less English support.
- - Unidades de Salud Mental Infanto-Juvenil (USMIJ) for under-18s — separate queue, usually faster.
- - Mutua sanitaria (private employer mutual) — many large employers (Fremap, Asepeyo, Ibermutua) offer psychology services; check with HR.
- - Telepsicología platforms: TherapyChat (Spanish), iFeel, Mindgram, Siquia — €40–60/session, Spanish + English options.
- - Colegio Oficial de Psicología of your CCAA has lists of certified psicólogos sanitarios.
- - Convenio Especial for non-SS insured: once registered, you can access SNS salud mental like any resident.
- - International Counseling Service (Barcelona, Madrid) — English-speaking counsellors, sliding scale.
- - Private Spanish psicólogos sanitarios: €50–90/session in-person; cities full of options.
Online Therapy
Insurance covered: No / partially
Private health insurance (Sanitas, Adeslas, DKV) usually covers 20 sessions/year of psicología with modest copays (€5–15). Check your policy — the network (cuadro médico) lists psicólogos. MAPFRE and AXA cover therapy on some plans. Online therapy via insurer platforms increasingly available. SNS does NOT yet fund private online therapy.
Expat-Specific Challenges
- -Culture shock curve — honeymoon (first 3 months), rejection (months 4–12), negotiation (year 2), mastery. The second autumn is typically hardest.
- -Isolation — Spaniards are warm but friendships build around shared activities (sport, fiestas, family) not quick small-talk. Need to show up repeatedly.
- -Administrative fatigue — NIE, empadronamiento, Hacienda, extranjería, homologación. The cumulative bureaucracy is exhausting — normalise that.
- -Language exhaustion — even good Spanish can be tiring daily. Regional languages (Catalan, Euskera, Galego, Valenciano) add another layer outside Castilian Spain.
- -Food & schedule adjustment — eating lunch at 14:30 and dinner at 22:00 disrupts sleep and hunger cycles for many newcomers. Give it 6 months.
- -Winter in Madrid / Meseta inland / Pyrenees can be surprisingly harsh and grey — SAD is real, especially for arrivals from sunnier countries expecting "sunny Spain".
- -Third-culture-kids dynamics — children pick up Spanish fast at escuela, parents lag behind, creating role inversion.
- -Distance from family — flights home are cheaper than Germany/UK but the emotional cost compounds; plan 1–2 yearly visits.
If you call 024 in crisis and need English, ask — operators are trained to transfer when possible. For immediate danger call 112. InterNations Spain and expat Facebook groups have active mental health threads with therapist recommendations. Medication: SSRIs and SNRIs are prescribed by psiquiatra through Receta Electrónica, with SNS copay 10–60% depending on income. If continuing a therapy or medication from abroad, bring a summary from your doctor in English/Spanish — avoids gap in treatment while you register with your new Centro de Salud.
