Healthcare in Italy

Italy's Servizio Sanitario Nazionale (SSN) is a universal public healthcare system, funded through general taxation and managed regionally by 20 ASL (Aziende Sanitarie Locali) or AUSL. Coverage is universal for all residents — Italian citizens, EU residents, non-EU residents with a valid Permesso di Soggiorno, and emergency care is also available to undocumented individuals (STP — Straniero Temporaneamente Presente card). The SSN is strong in primary and emergency care; specialist waiting lists can be long (weeks to months for non-urgent). There are significant quality variations between northern regions (Lombardia, Emilia-Romagna, Veneto — among the best in Europe) and southern regions (Campania, Calabria, Sicily — under-resourced and overburdened). Private healthcare (sanità privata) is widely used to bypass waiting lists.

Ranked 2nd in the 2000 WHO World Health Report. Italy has one of the highest life expectancies in Europe (82.9 years for men, 87.0 for women — among the top globally). OECD 2023: excellent primary and acute care outcomes. Main weaknesses: long specialist waiting times, regional disparities (north-south divide), underfunded mental health services, and limited dental coverage.

Public healthcare: Servizio Sanitario Nazionale (SSN)

National Health Service

Eligibility:Universal for all legal residents. EU citizens: register at Comune + ASL. Non-EU residents with Permesso di Soggiorno: register at ASL. Employed workers: registered via INPS employer notification. Undocumented individuals: access to emergency care via STP (Straniero Temporaneamente Presente) card, free from any ASL. Tourists from EU/EEA: covered by EHIC/EHIC for medically necessary treatment.

Monthly cost:Free at point of use for SSN-enrolled residents. Funded via general taxation (no dedicated health contribution in Italy — unlike Germany's GKV). For non-EU workers: INPS contributions fund the system indirectly. Voluntary SSN subscription for non-EU non-workers: see ASL — typically €387.35–€7,700/year depending on income and status (for those not covered through work/family). Students from non-convention countries: €149.77/year voluntary subscription.

Co-payments:Ticket sanitario: copayment for specialist visits and diagnostic tests. Amounts: €15–50 for specialist visits; €0–50 for diagnostic tests (depending on type). Exemptions (esenzione): income below €8,263/year (code E01/E02); chronic diseases (malattia cronica — codice malattia from ASL); pregnancy; disability; unemployment; age over 65 with low income (code E06). Check exemption codes at your ASL. Prescription drugs: fascia A (free or ticket €3–8.50/pack); fascia H (hospital only); fascia C (full price, not SSN). Ticket cap: €45.38 per prescription receipt.

How to enrol:1. Obtain your Codice Fiscale (from Agenzia delle Entrate or Italian consulate). 2. Register at your Comune (iscrizione anagrafica) — provides proof of Italian residency. 3. Visit your local ASL (Azienda Sanitaria Locale) with: Codice Fiscale, identity document, and proof of residency (certificato di residenza or receipt of iscrizione anagrafica application). 4. ASL issues your Tessera Sanitaria and assigns you to a medico di base (GP). 5. Register with your medico di base at the ambulatorio medico (GP surgery). 6. Children under 14: choose a pediatra di libera scelta (free-choice paediatrician) from the ASL list.

Waiting period:No waiting period for emergency care. For SSN enrollment: immediate upon registration. GP appointments: typically available within 1–5 days. Specialist appointments (SSN): can be 2–6 months for non-urgent. CUP (Centro Unico Prenotazioni) booking system — book via phone, online, or at ASL offices.

Private healthcare: Sanità Privata (Private Healthcare)

Who can use it:Open to all. Widely used by all income levels to bypass SSN waiting times. Many employers offer mutua sanitaria integrativa (private supplementary insurance) as an employment benefit.

When it makes sense:Highly recommended for: faster specialist access (same-day vs months on SSN); dental care (virtually all dental is private in Italy); physiotherapy without long waits; psychological therapy (SSN waits are very long); regions with weaker SSN infrastructure (southern Italy). Many working Italians have employer-provided mutua (Unisalute, Previmedical, Fasi, etc.).

Monthly cost:€50–120/month (young adult, basic plan); €100–200/month (middle-aged); €200–500/month (over 60 or pre-existing conditions). Family plans: €200–400/month. Employer mutua: often free or heavily subsidised.

Accessing care

GP system

Every SSN-registered resident is assigned a medico di base (family doctor / medico di medicina generale). You can choose your GP from the ASL list. GPs work with lists of up to 1,500 patients. Appointments typically available within 1–5 days; urgent cases seen same-day. The GP acts as gatekeeper for specialist referrals and hospital admissions. Many GPs now offer phone or video consultations. Find your GP surgery (ambulatorio) via the ASL website or the IO app.

Specialist referral

For SSN specialist care, you need an impegnativa (referral) from your medico di base. Book via CUP (Centro Unico Prenotazioni) — online, by phone, or in person at ASL. Waiting times: 2–6 months for non-urgent specialists (ortopedico, dermatologo, oculista); 2–4 weeks for urgent (marcato P or urgente on the referral). Priority codes: U (urgente, 72h), B (breve, 10 days), D (differibile, 30 days), P (programmabile, 60–120 days). Private specialists: direct booking, €80–250/visit, typically within days.

Emergency care

Pronto Soccorso (A&E) at public hospitals 24/7 — free at point of care, with triage system (codice bianco/verde/giallo/rosso). For life-threatening emergencies: 118 (ambulanza) or 112. For non-urgent out-of-hours: Guardia Medica (see below). Hospitals operating Pronto Soccorso are publicly listed by region — know your nearest hospital before you need it.

Out of hours

Guardia Medica / Continuità Assistenziale: out-of-hours GP service operated by ASL. Evenings (20:00–08:00), weekends (Sat 08:00–Mon 08:00), and public holidays. Home visits available. Find your local number on the ASL website or dial 116117 (pan-EU non-emergency medical). For urgent medical issues: go to Pronto Soccorso directly.

Prescriptions

GP issues prescriptions (ricette) electronically via the Ricetta Elettronica Dematerializzata (RED) system. You receive an NRE (Numero Ricetta Elettronica) code and collect at any farmacia with your Tessera Sanitaria. Repeat prescriptions for chronic conditions can be managed via the Fascicolo Sanitario Elettronico app. Copayment depends on income and exemption status.

Dental

SSN dental care for adults is almost nonexistent — only emergency extractions, acute infections, and specific vulnerable groups (pregnant women, immunocompromised patients) receive free or subsidised dental care. Children 0–14: some regions offer free preventive dental care. Most adults pay privately: check-up €60–100; cleaning €80–120; filling €80–150; implant €1,200–2,500; orthodontics €3,000–8,000. Dental insurance from mutue integrative covers basic dental care. CAF (Centro Assistenza Fiscale) dental cost deductions: 19% tax deduction on dental expenses declared in Modello 730.

Mental health

SSN mental health is managed by DSM (Dipartimento di Salute Mentale) and CSM (Centro di Salute Mentale) at the local ASL. CSM provides: psychiatry appointments (psichiatria), community mental health support, and access to day hospitals. Waiting times are extremely long for psychology (psicologia clinica): 6–18 months typical. Bonus Psicologo (Psychological Support Bonus): €500/year (€1,500 for ISEE under €15,000) from INPS for private psychology sessions — introduced 2022, confirmed for 2026. Apply via INPS portal in March–April. Private psychologists/therapists: €60–120/session; increasingly online (Better Help, Serenis in Italian).

Maternity

Excellent SSN maternity coverage. All prenatal care (consulti ostetrico-ginecologici), ultrasounds, blood tests, and childbirth are fully covered. Libretto della gravidanza provides free check-ups schedule. Consultori Familiari (family health centres) offer midwifery support and parenting classes. Birth in public hospitals: free. Post-partum care at home by ostetrica (midwife): available via ASL. Maternity leave (congedo di maternità): 5 months at 80% salary via INPS.

Notable English-speaking healthcare in Rome: Salvator Mundi International Hospital (+39 06 588961), Rome American Hospital (+39 06 22551), Clinica Paideia. In Milan: Humanitas Clinical and Research Centre (Rozzano), Istituto Clinico Humanitas, Auxologico. In Florence: International Medical Centre on Via Porta Rossa. For GP: some expat-area practices advertise English services — check expat community listings.

International insurance:International health insurance recommended for: visa applicants requiring proof of coverage (elective residency, digital nomad), those waiting for SSN enrollment, and those wanting consistent English-speaking medical access. Providers: Cigna Global, Allianz Care, AXA International, Foyer Global Health, IMG Global. Monthly cost: €100–300 for comprehensive international coverage. Note: private Italian health insurance (mutua sanitaria) is supplemental, not a substitute for SSN enrollment.

Emergency card info:Carry your Tessera Sanitaria at all times — required at all medical appointments. EU residents: also carry your EHIC card as backup. Memorise 118 (ambulance) and 112 (all emergencies). The Fascicolo Sanitario Elettronico (electronic health record) is accessible at fascicolo.salute.gov.it — register with SPID for digital access to all your Italian medical records, prescriptions, and lab results.

EHIC notes:EU/EEA/Swiss citizens: EHIC (or GHIC for UK) covers medically necessary care during visits to Italy but is NOT a substitute for SSN registration as a resident. Once you register your residency at the Comune, you must enrol in the SSN. Long-term Italian residents should enrol in SSN rather than relying on EHIC.

Pharmacies (Farmacia)

Hours:Typically Mon–Fri 08:30–13:00 and 15:30–19:30; Sat 08:30–13:00. Many large pharmacies (farmacie aperte) maintain continuous hours. 24-hour rotating duty (farmacia di turno): posted outside every farmacia and at farmaciaditutrno.it or the IO app.

After-hours / emergency:Farmacia di turno (24h duty pharmacy) on a rotating basis within each district. Look for the illuminated green cross outside the pharmacy or check the app. Night surcharge typically €2–4.13 (diritto di chiamata notturna). Some major cities have dedicated 24h farmacie.

Prescription rules:Farmaci di fascia A (SSN subsidised, green stripe on box): present Tessera Sanitaria + electronic prescription NRE code. Farmaci di fascia C (not SSN): full price, no prescription required for OTC. Farmaci con ricetta (prescription required): red stripe. Antibiotics, sleeping pills, and controlled substances require a white prescription (ricetta bianca) from GP. Some antibiotics are technically OTC in Italy but pharmacist discretion varies.
