Healthcare in Iceland

Iceland operates a contributory public healthcare system funded through a combination of general taxation, employer/employee social insurance contributions (tryggingagjald), and co-payments. Sjúkratryggingar Íslands (SÍ — Icelandic Health Insurance) is the main public health insurer, administering benefits and co-payment reimbursements. Healthcare is generally of excellent quality — Iceland consistently has one of the world's lowest infant mortality rates and longest life expectancies. The system is a modified universal system: residents are automatically enrolled once they have a kennitala and have paid into the system for the required period. Co-payments apply to most services but are moderate compared to private costs.

Icelandic Health Insurance

Eligibility:All legal residents of Iceland who have been registered with kennitala and have lived in Iceland for at least 6 months (or are employed here). EEA workers are covered from day one of employment. There is a 6-month waiting period for some benefits for new residents not in employment. Interim coverage: EHIC card or travel insurance during the waiting period.

Monthly cost:No separate monthly premium — funded through general taxes and the tryggingagjald social insurance contribution (employee 4%, employer 6.35%). Co-payments apply per visit/service.

Co-payments:Co-payments apply to most healthcare services. GP visit: approximately ISK 500 (standard working-hours visit, 2026). Specialist visit: ISK 1,500–3,000. Emergency care: ISK 2,000–5,000. Inpatient stay: daily fee of approximately ISK 1,500–2,500 (waived after 28 days). Monthly maximum co-payment cap (37,794 ISK for adults in 2026) — once reached, further services are free for the rest of that month. Prescription drugs: graduated subsidy — the more you spend per year, the higher the state contribution. Children under 18 and certain vulnerable groups pay less.

How to enrol:1. Register at Þjóðskrá Íslands (National Registry) and get a kennitala. 2. This automatically registers you with Skatturinn for tax. 3. Apply for registration with Sjúkratryggingar Íslands (SÍ) at sjukra.is — or via island.is. 4. Choose a primary care practice (heilsugæslustöð) near your home. 5. You are then in the system and can book GP appointments.

Waiting period:6 months for some non-emergency benefits if not employed. Emergency care and maternity care available immediately for all residents. Workers covered immediately from first day of employment.

Private healthcare: Private healthcare

Who can use it:Anyone can access private healthcare in Iceland — no referral needed for most private specialists.

When it makes sense:For faster specialist access, bypassing queues for elective procedures, private dental care, private physiotherapy, and for international health insurance if your employer provides it. Reykjavik has several private clinics and private specialist practices.

Monthly cost:Employer-provided international health insurance: typically included for senior expat hires. Private consultations: ISK 20,000–60,000 for specialist visits. Dental: ISK 15,000–80,000 per procedure.

Accessing care

GP system

The primary care system in Iceland uses heilsugæslustöðvar (community health centres). These are your first point of contact for non-emergency healthcare. Residents register with a heilsugæslustöð in their area. GP (heimilislæknir — family doctor) appointments can be booked online at heilsugaeslan.is or by phone. In Reykjavik, the main community health centres serve different districts.

Specialist referral

GP refers you to a specialist (sérfræðingur). You can also self-refer to private specialists for a fee. Public specialist waiting times vary — typically 4–16 weeks for elective referrals. Emergency referrals are processed rapidly. Landspítali (the National University Hospital of Iceland) is the main tertiary hospital.

Emergency care

Emergency (lífshættuleg): call 112 immediately. Landspítali University Hospital has the main emergency department (bráðamóttaka). For non-life-threatening urgent care, Heilsugæslan (community health) can also help during opening hours.

Out of hours

Læknavaktin (1770) — the out-of-hours medical service. Call 1770 first — a nurse triages your issue by phone. Open evenings, nights, weekends, and holidays. Based at Smáratorg for physical visits in the capital area.

Prescriptions

Prescriptions are issued electronically by your GP. Collect from any apotek (pharmacy). The lyfjagreiðsluþátttaka system subsidises prescription costs — show your kennitala at the pharmacy. Generic substitution is encouraged.

Dental

Adults: dental care is largely private and expensive in Iceland. A standard check-up: ISK 12,000–20,000. Complex treatment: ISK 50,000–300,000+. Children under 18: dental care is covered or heavily subsidised through the public system. There is partial subsidy for adults via SÍ for specific procedures. Join private dental insurance through your employer or trade union if available.

Mental health

Public mental health: GP referral to Geðsvið Landspítala (Psychiatric Services, Landspítali) for more serious conditions. Waiting times for non-urgent psychological support: 3–9 months in the public system. Iceland has some of the world's highest antidepressant use per capita — partly a response to extreme seasonal darkness. Private psychologists: ISK 20,000–35,000/session. Sálfræðingafélag Íslands (Icelandic Psychological Society) has a directory of licensed practitioners.

Maternity

Excellent and largely free. Midwife (ljósmóðir) care throughout pregnancy. Landspítali and selected regional hospitals handle births. Fæðingarorlof (parental leave): 6 months per parent (each) + 6 months shared = 18 months total between parents (as of 2024 reforms). This is among the most generous parental leave systems in the world.

Finding English-speaking doctors:English is very widely spoken by Icelandic medical professionals — almost all doctors and nurses at Landspítali and most community health centres speak excellent English. The international community in Iceland means English-language healthcare is readily available. Ask reception to confirm if needed.

International insurance:If you are on an expat package, your employer may provide international health insurance (e.g., Cigna, Allianz Care, AXA PPP). This can provide faster specialist access and international treatment coverage. For general residents, the public system is sufficient for most needs.

Emergency card info:Keep your kennitala memorised — it is your key to the healthcare system. Iceland issues a SÍ card (sjúkratryggingakort) that you should carry. For emergency care, giving your kennitala is sufficient for staff to access your records.

EHIC notes:Iceland participates in the European Health Insurance Card (EHIC) system. Apply for an Icelandic EHIC at sjukra.is. Valid for temporary stays needing medical care in EU/EEA countries. For travel within the EEA, always carry your EHIC.

Pharmacies (Lyfjabúð / Apótek)

Hours:Main pharmacies in Reykjavik: Monday–Friday 09:00–18:00 (some until 19:00 or 20:00); Saturday 10:00–16:00. Sundays: most closed or reduced hours. Lyfja (the largest Icelandic pharmacy chain) has multiple locations. Lyf & heilsa is another major chain.

After-hours / emergency:Out-of-hours pharmacy service: Lyfja at Háaleitisbraut in Reykjavik and the pharmacy at Læknavaktin (Smáratorg) provide emergency dispensing. Information at lyfjavakt.is.

Prescription rules:Electronic prescriptions (rafræn lyfseðill) — give your kennitala to the pharmacist. Subsidy through lyfjagreiðsluþátttaka system reduces costs for frequent medication users. OTC medicines: paracetamol (parasetamól), ibuprofen, antihistamines available without prescription. Some OTC drugs require purchase from a pharmacist rather than supermarket.
