Healthcare in Puerto Rico
Puerto Rico operates a mixed public-private healthcare system under US federal frameworks. Medicaid (Mi Salud) covers approximately 70% of the population. Medicare covers eligible seniors and disabled individuals. Private insurance — through employers or the ACA Marketplace — covers working professionals and Act 60 expats. Public hospitals exist island-wide but are often overcrowded. Private hospitals (Auxilio Mutuo, Ashford Presbyterian, Hospital San Jorge) offer US-standard care. Accessing care
Puerto Rico does not have a formal GP referral gatekeeper system for private insurance. With ACA or employer plans, you can see a specialist directly, though in-network referrals may be required to avoid higher out-of-pocket costs. Primary care physicians (médicos de cabecera) are widely available in San Juan metro. For Mi Salud patients: you are assigned a primary care doctor through your Mi Salud plan.
For private insurance (ACA/employer): most plans allow direct specialist access for in-network providers. Check your plan's network directory. For Mi Salud: a primary care referral is typically required for specialists. Wait times for specialists in the public system can be weeks to months.
Call 911 for life-threatening emergencies. Puerto Rico Medical Center (Centro Médico in Río Piedras) is the Level 1 trauma centre. Major private emergency rooms: Auxilio Mutuo (787-758-2000), Ashford Presbyterian (787-721-2160), HIMA San Pablo. Emergency rooms are accessible regardless of insurance status under EMTALA (US federal law). Out-of-pocket ER costs without insurance: $500–2,000+.
Urgent care centres (clínicas de cuidado urgente) are available across San Juan metro for non-emergency after-hours issues — typically $120–200 without insurance. Many Walgreens locations have health clinics. Telemedicine (Teladoc, MDLive) is widely used and accepted by most PR insurance plans — access doctors 24/7 via app.
US prescriptions are valid in Puerto Rico at Walgreens, CVS, and local pharmacies (Farmacia del Pueblo). GoodRx discounts apply in Puerto Rico at chain pharmacies. Generic medications are significantly cheaper than brand-name. Some medications available in PR that require a prescription in the US mainland. Most PR plans require 90-day supply for maintenance medications.
Dental care is not covered by standard ACA health insurance plans. Separate dental insurance is recommended — average dental plan: $20–50/month. Without insurance: dental cleaning $80–150, filling $150–300. Dental tourism to the Dominican Republic is popular among PR residents for major procedures (crowns, implants at 30–50% lower cost). C-section rate in PR is high by US standards (~50%). For expats: ensure your private plan covers maternity from the start — some plans have waiting periods of 10–12 months for maternity benefits.
Expat-specific advice
- Obtain private health insurance before or immediately upon arriving in Puerto Rico — do not go uninsured even for a day.
- If arriving during ACA open enrollment (November–January): enroll at healthcare.gov. If outside open enrollment: apply within 60 days of establishing PR residence (qualifying life event).
- Register with a primary care physician (PCP) in your insurance network within the first month.
- Bring a 90-day supply of any prescription medications — allow time to establish care with a PR physician before your supply runs out.
