Thailand has endemic rabies — any bite, scratch, or lick from a dog, monkey, or bat requires same-day medical attention

Rabies is endemic throughout Thailand. Street dogs are ubiquitous in every city, town, and village — including tourist areas (Chiang Mai, Pai, Koh Samui, Phuket, Bangkok). Monkeys at temple sites (Lopburi, Phra Prang Sam Yod), monkey feeding areas on islands, and forest parks can scratch and bite. Bats in rural areas and caves also pose a risk. Any bite that breaks the skin, any scratch (even through clothing), or any lick on a mucous membrane or broken skin from ANY mammal constitutes a potential rabies exposure and must be treated as an emergency. Post-exposure prophylaxis (PEP): immediately wash the wound with soap and water for at least 15 minutes (this alone dramatically reduces viral load), then go to the nearest major hospital that day — Bumrungrad (Bangkok), Samitivej (Bangkok), Bangkok Hospital group locations, McCormick Hospital (Chiang Mai), or the nearest provincial district hospital. PEP consists of a vaccine series (days 0, 3, 7, 14) and, for patients not previously vaccinated, Rabies Immunoglobulin (RIG) injected into the wound on day 0. RIG supply at smaller clinics can be limited — major hospitals stock it reliably. Pre-exposure prophylaxis (3 doses before travel) eliminates the need for RIG, significantly simplifying post-exposure management.

⚡ Rabies is 100% fatal once neurological symptoms appear. A decision to "wait and see" after a dog bite in Thailand is life-threatening. Do not allow reassurances that the dog "looks healthy" — rabid animals can appear normal up to 10 days before death.

✅ Consider pre-exposure rabies vaccination (3 doses over 21 days) before living in Thailand long-term, especially if you have children or spend time in rural areas. After any animal bite or scratch: wash immediately with soap and water for 15 minutes, then go to a hospital the same day — no exceptions. Never approach or feed street dogs or monkeys. If a dog or monkey is near you: stand still, do not run (triggers chase instinct), avert eye contact, and slowly back away.
