Healthcare in Chile

Chile operates a dual public-private healthcare system. All employed workers contribute 7% of their gross salary to either FONASA (Fondo Nacional de Salud, public) or an ISAPRE (Institución de Salud Previsional, private). FONASA covers approximately 80% of the population and provides comprehensive coverage via public hospitals and contracted private providers. ISAPRE covers about 15% — mostly higher-income earners seeking shorter waiting times and more comfortable facilities. Private hospital care (Clínica Alemana, Clínica Las Condes) is world-class and significantly faster than the public system. Quality varies dramatically: public hospitals in Santiago are generally adequate; rural public facilities can be very basic.

Chile ranks approximately 33rd globally (WHO-aligned assessments); best public healthcare in South America with Santiago having excellent specialist care in both public and private sectors.

Public healthcare: FONASA — Fondo Nacional de Salud

National Health Fund

Eligibility:All employed workers who do not choose an ISAPRE, as well as unemployed, pensioners, and low-income individuals. Foreign residents with a valid visa and RUT can enroll. Dependants (spouse, children under 18) are covered under the contributor's FONASA membership at no additional charge.

Monthly cost:7% of gross salary (employee-only contribution — employer does not pay health contribution in Chile). For Group A/B (indigent/low income): state-subsidised. Minimum monthly contribution: approximately CLP $37,000 (based on minimum wage). Self-employed: pay 7% of declared boleta income.

Co-payments:FONASA has 4 groups based on income: Group A (indigent, free) — no copayment for any public service. Group B (income up to ~1.7 minimum wages, ~CLP $900,000/month) — free at CESFAM and public hospitals. Group C and D (higher income) — pay 10–20% co-payment for public hospital services. The Modalidad de Libre Elección (MLE) allows FONASA members to see private contracted providers at a discount — FONASA covers a fixed amount and the patient pays the difference (can be CLP $10,000–50,000+ per visit).

How to enrol:Enroll at any FONASA office (Atención al Cliente) or online at fonasa.cl. Bring: RUT, cédula de identidad, employment contract or proof of income. Self-employed on boleta de honorarios: enroll with proof of AFP contributions. Unemployed/low income: apply for Group A or B classification. Enrollment is immediate but the first CESFAM appointment booking can take 1–4 weeks.

Waiting period:No waiting period for emergency care. CESFAM primary care: typical wait 1–4 weeks for appointment. Public specialist referral: can be 2–12 months for non-GES conditions. GES (Garantías Explícitas en Salud) guarantees maximum wait times for 87 priority conditions — if exceeded, FONASA must fund private care.

Private healthcare: ISAPRE — Institución de Salud Previsional

Who can use it:Any employed worker choosing to opt out of FONASA; self-employed who can demonstrate income. Must pay minimum 7% of salary; many plans require additional voluntary top-up for better coverage.

When it makes sense:When you value shorter wait times, choice of doctor, private hospital rooms, and higher-quality facilities. Most useful for: families with children, those with chronic conditions requiring frequent specialist visits, and higher-income workers (Groups C/D under FONASA). Compare carefully — some ISAPREs have complex fine print and high premium hikes with age.

Monthly cost:Minimum 7% of salary (same as FONASA) + voluntary top-up. Average total: CLP $120,000–350,000/month for an individual. Family plans significantly higher. Premiums increase with age. Compare at supersalud.gob.cl.

Accessing care

GP system

FONASA: Register at your nearest CESFAM (Centro de Salud Familiar) with your RUT and cédula. The CESFAM serves as your GP clinic and first point of contact for all health needs. CESFAM appointments can be made in person or via the FONASA app/website. ISAPRE: Choose any doctor in your ISAPRE's network — no GP gatekeeping required for many specialist consultations. Private GP consultations: CLP $20,000–50,000 out of pocket at any clínica.

Specialist referral

FONASA: GP at CESFAM refers to public hospital specialist — wait times vary from weeks to many months for non-GES conditions. GES guarantees (87 diagnoses including cancer, cardiac, diabetic, mental health) have maximum wait times enforced. ISAPRE: Direct specialist access within network; wait times days to weeks. Emergency: direct to nearest hospital urgencias (emergency department) regardless of FONASA/ISAPRE status.

Emergency care

Emergency services (SAMU) available via 131 (call from any mobile, free without credit). Hospital Urgencias: all public and private hospitals have emergency departments. Emergency care cannot be refused regardless of FONASA status or insurance. Major public emergency hospitals in Santiago: Hospital del Salvador, Hospital de Urgencias de la Asistencia Pública (HUAP, Posta Central). For life-threatening emergencies, call 131 — SAMU dispatches ambulances throughout Chile.

Out of hours

CESFAM operates weekdays 8am–5pm. For out-of-hours primary care (not emergencies): Urgencias in public hospitals (24/7). Some ISAPREs have 24/7 medical call lines. Farmacia de turno (duty pharmacy) list available at MINSAL or posted at pharmacies — one pharmacy per commune stays open 24h on rotation.

Prescriptions

Prescriptions issued by licensed doctors. Controlled substances (psychoactive, opioids) require official numbered prescription (Receta Médica Retenida) from controlled prescription pad. Medications are generally much cheaper in Chile than Western Europe or North America. Generic medications available at Cruz Verde, Salcobrand, and Farmacias Ahumada at significant discounts. Some medications available without prescription that require prescription elsewhere.

Dental

Not covered by FONASA for adults beyond basic preventive check-ups. Children under 20 covered by SEMÁFORO dental programme at CESFAM. Private dental care is affordable by international standards: consultation CLP $20,000–40,000, cleaning CLP $25,000–50,000, filling CLP $30,000–80,000. Some ISAPREs include dental cover as an add-on — check your plan. Dental schools (Universidad de Chile, UACH, UC) offer subsidised care by supervised students.

Mental health

Public mental health via COSAM (Centro Comunitario de Salud Mental) attached to CESFAM networks — free for FONASA patients but significant wait times (4–12 weeks for initial assessment). GES guarantees cover: severe depression, bipolar disorder, schizophrenia, substance abuse, and childhood/adolescent mental health. Private psychologists: CLP $30,000–80,000/session. Private psychiatrists: CLP $50,000–120,000. Línea de Salud Mental: 800 104 024 (24/7, free).

Maternity

Maternity care is comprehensive under both FONASA and ISAPRE. FONASA Groups A and B: fully free including delivery. Public hospital births: well-managed and free. Private hospital births at Clínica Alemana or Clínica Las Condes: CLP $2,000,000–6,000,000 total. Prenatal checks: every 4 weeks at CESFAM for FONASA patients, starting from confirmed pregnancy. Midwife (matrona) system is strong in Chile — matronas handle most low-risk deliveries.

Expat-specific advice

First steps on arrival:

Enroll in FONASA immediately on receiving your RUT and cédula — visit the nearest FONASA office with your cédula and employment contract.
Register at your closest CESFAM for primary care — the registration is commune-specific; choose the CESFAM nearest your address.
Keep the FONASA card and your cédula together — present both at all medical appointments.
If you choose ISAPRE: compare plans at supersalud.gob.cl; negotiate the premium and coverage before signing (prices are negotiable); ask specifically about pre-existing conditions exclusions.

Finding English-speaking doctors:English-speaking doctors are common at Clínica Alemana de Santiago (has a specific international patient unit), Clínica Las Condes, and Clínica Universidad de los Andes. Integramédica and Bupa Chile also have English-speaking specialists. At public CEAFAMs and public hospitals, English is rarely spoken. Expatriate Facebook groups (Santiago Expats, Expats in Chile) maintain updated lists of English-speaking doctors by specialty.

International insurance:International health insurance (Cigna Global, AXA Expatriate, Allianz Care, Bupa International) is accepted at all private hospitals and many specialist clinics in Santiago. Pre-authorization is typically required for non-emergency hospitalisation. Some international policies exclude pre-existing conditions for the first 12 months — check your policy. International insurance is particularly valuable for coverage during the first 30–90 days before FONASA or ISAPRE enrollment is complete.

Emergency card info:Carry your RUT and FONASA/ISAPRE card at all times. If hospitalised, present your RUT — all medical records in Chile are linked to the RUT. Emergency: 131 (SAMU ambulance). Chilean emergency services are well-equipped in major cities; rural areas have more limited capacity. For non-life-threatening emergencies, the Urgencias section of any hospital accepts walk-ins.

Pharmacies (Farmacia)

Hours:Major chains (Cruz Verde, Salcobrand, Farmacias Ahumada) open daily 9am–10pm or later. Shopping mall pharmacies often open until 10pm or midnight. Farmacia de turno (duty pharmacy): one per commune, open 24h — the rotating schedule is posted at pharmacies and available via MINSAL.

After-hours / emergency:Farmacia de turno system: each commune rotates one open-24h pharmacy. Find your duty pharmacy at farmaciasdeturno.cl or the MINSAL website. Duty pharmacies are required by law to dispense essential medications. After-hours premium may apply.

Prescription rules:Most common medications (antibiotics, blood pressure medications, common pain medications) dispensed without prescription. Controlled substances require numbered official prescriptions — retained by the pharmacy. Psychotropic medications require special "receta médica retenida." Bring medications in original packaging with generic names listed (international generic names are accepted). Importing medications: bring a letter from your doctor for any unusual or controlled substance — present at Customs/SAG.
