Healthcare in Uruguay

Uruguay operates the SNIS (Sistema Nacional Integrado de Salud), a mixed public-private health system established in 2007. Formal sector employees and their dependants access healthcare through FONASA contributions, which finance membership at a mutualista (private health cooperative). The public ASSE (Administración de los Servicios de Salud del Estado) system covers those without FONASA coverage. Quality is reasonable by Latin American standards, with Montevideo having the strongest facilities. Complex cases may still require referral to Argentina or Brazil.

Ranked 35–40 in Latin American WHO metrics. Best public health indicators in South America by most measures. Montevideo public/private hospitals are significantly better than interior facilities.

Public healthcare: ASSE / Mutualistas through FONASA

Administración de los Servicios de Salud del Estado / National Health Fund-financed mutual cooperatives

Eligibility:All formal sector employees, self-employed BPS contributors, retirees, and their registered dependants (spouse + children under 18) are covered by FONASA. Informal workers and non-contributors use ASSE directly.

Monthly cost:3–8% of salary deducted at source (employee FONASA contribution). Employer pays additional 5%. No separate premium to the mutualista — FONASA finances the entire mutualista membership.

How to enrol:Formal employees are registered by their employer with BPS and FONASA. New employees should confirm with HR that BPS registration has been completed. Choose your mutualista at BPS enrollment — most expats in Montevideo choose Hospital Británico mutual, Casmu, Mucam, Hospital Español, or Summum. Self-employed workers register directly at BPS (bps.gub.uy). First mutualista visit requires presenting cédula de identidad and FONASA credential.

Waiting period:30 days from BPS registration before full mutualista coverage activates. Emergency care covered immediately.

Private healthcare: Private Consultations Outside FONASA

Who can use it:Any resident or visitor can access private consultations at private clinics and hospitals, independent of FONASA coverage.

When it makes sense:For faster access to specialists, English-speaking doctors, second opinions, or procedures not covered by mutualista. Hospital Británico (private wing) is the top choice for English-speaking expats. Also useful for dental, optical, and aesthetic procedures.

Monthly cost:Private consultations: $U 1,800–4,000 per visit. International health insurance (expat plans): USD 100–350/month. Standalone dental insurance: USD 40–100/month.

Accessing care

GP system

Choose a médico de cabecera (GP/family doctor) at your mutualista when enrolling. All specialist referrals are typically made through your GP. In practice, many mutualistas allow direct specialist appointments for common conditions. Turno (appointment) system — book by phone or app. Average wait: 3–10 days for GP, 2–6 weeks for specialist.

Specialist referral

GP referral required for specialists within the mutualista system (some exceptions for gynaecology, ophthalmology). Out-of-network specialist visits not covered. Emergency departments at mutualista operate without referral. For private specialists outside your mutualista — go direct, no referral needed but full cost applies.

Emergency care

Call 105 for SAME (Sistema de Atención Médica de Emergencia) ambulance or go to your mutualista's emergency centre. Emergency care is available immediately for all with FONASA. ASSE emergency departments serve the general public. Hospital de Clínicas (public teaching hospital) handles complex trauma. 911 dispatches police + SAME + fire.

Out of hours

Mutualistas run 24/7 emergency centres (emergencia y urgencia). Call your mutualista emergency line directly for faster dispatch of their ambulance. SAME 105 is the public emergency service. After-hours pharmacy: rotating farmacia de turno system — check at farmashop.com.uy or ask any pharmacy.

Prescriptions

GP issues recipes (prescriptions) at the mutualista. Prescriptions filled at mutualista pharmacy at discounted rates. Some medications require specialist prescription (controlled substances, biological therapies). Generic medications available and affordable. Farmacias Devoto, Cruz Verde, and Farmashop are the main commercial chains.

Dental

Dental care is largely excluded from standard FONASA/mutualista coverage. Private dental plans available through MUCAM dental, Médica Uruguaya, and some employers. Basic extraction at public ASSE dental centres available free. Private dental consultation: $U 1,200–2,500.

Mental health

Mutualistas provide up to 15 psychology sessions per year and psychiatry referrals. Mental Health Law (2017) mandated full integration of mental health into SNIS. Public ASSE mental health centres available for low-income users. Private psychologists charge $U 1,500–3,000/session. Waiting times for public mental health: 4–12 weeks.

Maternity

All maternity care (prenatal visits, delivery, postnatal) covered by FONASA/mutualista. Delivery at mutualista hospital with obstetrics team. Choice of partner present during delivery. Postpartum home visit by midwife (partera) available. C-section rates are high in private sector (~40%). All newborns automatically enrolled in FONASA if parent is a registered contributor.

First steps on arrival:

Confirm BPS registration with your employer within your first week — FONASA coverage starts after 30 days.
If self-employed, register at BPS as trabajador no dependiente immediately to access FONASA.
Choose your mutualista at BPS registration — Hospital Británico Mutual is recommended for English-speaking expats in Montevideo.
Get your carnet de salud laboral (occupational health card) at your mutualista within 30 days of starting work.
Register as a patient with a médico de cabecera (GP) at your chosen mutualista.
Keep international health insurance active for the first 30-day FONASA waiting period.

Finding English-speaking doctors:Hospital Británico (Montevideo) has English and German-speaking doctors. Many senior doctors at Casmu, Hospital Español, and private clinics speak English. The British community has maintained Hospital Británico since 1857 — it is the standard recommendation for English-speaking expats. Hospital Italiano serves Italian-speaking expats. CLIAP (private clinic network) has English-speaking specialists.

International insurance:International health insurance is strongly recommended during the first 30 days before FONASA activates, and as a supplement for complex procedures, medical evacuation, and dental/optical care that mutualistas do not cover. Bupa, Cigna Global, AXA, and Allianz offer expat plans covering Uruguay. Medical evacuation to Buenos Aires or São Paulo for complex cases costs USD 15,000–50,000 — ensure evacuation is included.

Emergency card info:Save your mutualista emergency phone number in your phone immediately on enrollment. SAME: 105. General emergency: 911. Carry your cédula de identidad and FONASA mutualista credential card at all times — hospitals require them for rapid processing.

Pharmacies (Farmacia)

Hours:Monday–Saturday 08:00–22:00 typically. Rotating 24h on-call (farmacia de turno) — one pharmacy per neighbourhood is open overnight and on Sundays. Check turno at farmashop.com.uy or ask locally.

After-hours / emergency:Farmacia de turno: rotating 24h emergency pharmacy service. Most pharmacies post the current turno location in their window. Farmashop app shows nearest open farmacia in real time.

Prescription rules:Most medications require a recipe (prescription) issued by a licensed Uruguayan doctor. Exceptions: many common antibiotics, antihistamines, and OTC pain relief sold without prescription. Controlled substances (benzodiazepines, opioids) require special triplicate prescriptions. Foreign prescriptions not accepted — consult a Uruguayan doctor for any ongoing medication need.
