Estonia operates a Bismarck-model social health insurance system (funded primarily through employer social tax). The Estonian Health Insurance Fund (Eesti Haigekassa) covers approximately 95% of the population. Haigekassa is funded through the employer social tax (33% of gross salary, of which 13% goes to pension and 20% to health insurance). There is no separate employee health insurance premium. Healthcare quality is good and improving — Estonia invested heavily in health IT (e-prescription, digital patient records, e-ambulance) and ranks well on digital health innovations. Accessing care

GP system

Every Haigekassa-covered resident should register with a perearst (family doctor). Find available doctors at perearst.ee. Registration is free — phone the practice or register online at some clinics. Your perearst is your healthcare gatekeeper — referrals to specialists flow through them. Most perearstid have a small list size and know their patients personally. GPs in Tallinn and Tartu often speak English.

Specialist referral

Your perearst (GP) issues a suunamine (referral) for specialist treatment. The referral is sent digitally — you book an appointment directly with the specialist clinic using your referral code. You have the right to choose which clinic (within the list of Haigekassa-contracted providers). Specialist visit co-payment: €5 per visit.

Emergency care

Emergency (life-threatening): call 112 — kiirabi (ambulance) services are excellent and free. Emergency department (erakorraline vastuvõtt): for urgent but non-life-threatening conditions. Main emergency hospitals in Tallinn: North Estonia Medical Centre (PERH), East Tallinn Central Hospital (ITKÜM). Show your ID card — your insurance and medical history are digitally accessible.

Out of hours

Outside GP hours, call 1220 — the national medical advice line (arsti nõuandeliin). A nurse triages your condition and advises whether you need emergency care or can wait. For non-life-threatening urgent care: duty clinic (valvepolikliinik) — your GP practice's answering service will direct you. Emergency dental: dental duty services (valve hambaarst) in Tallinn operate evenings/weekends — check specific arrangements via 1220.

Prescriptions

Prescriptions are issued electronically (e-retsept) by your doctor. You collect at any apotek (pharmacy) — show your ID card and the pharmacist retrieves your prescription digitally. Reimbursed medications: show your ID card. Check your prescriptions and reimbursements at haigekassa.ee.

Dental

Adults (19+): dental care is substantially private. Haigekassa subsidises up to ~€30/visit for insured persons — largely symbolic. Expect to pay €30–200 per procedure out of pocket. Dental check-up: €30–60. Filling: €40–100. Many Estonians combine dental trips abroad (Latvia, Finland) for cost savings. Mental health

Public: GP referral to psychiatrist (psühhiaater) — free. Maternity

Fully covered by Haigekassa. Prenatal care, midwife (ämmaemand) services, delivery at maternity hospital (sünnitushaigla) — all free. Choice of hospital for birth. Parental benefit (vanemahüvitis) is a separate social insurance benefit paid by Sotsiaalkindlustusamet.

Finding English-speaking doctors:Confido clinic and Medicover in Tallinn have excellent English-speaking staff. Many GPs at major Tallinn practices speak English. Tartu University Hospital (TÜKH) has international patient services. Outside major cities, English is less reliably available — bring translation support or use Google Translate for consultations.

International insurance:International health insurance from your employer may supplement Haigekassa coverage for private treatments, dental, optical, and faster specialist access. Check if your international insurer recognises Haigekassa coverage to avoid double-paying. Travel insurance should cover emergency repatriation if needed.

Emergency card info:Show your Estonian ID card at any medical facility — your Haigekassa status, blood type, known conditions, current medications, and medical history are accessible digitally in the nationwide Tervise Infosüsteem (TIS — Health Information System). This is one of Estonia's most impressive digital health achievements.

EHIC notes:Your Estonian EHIC (European Health Insurance Card) is issued by Haigekassa — apply at haigekassa.ee. Valid for emergency and necessary treatment in other EU/EEA countries and Switzerland. Processing: approximately 2 weeks. The card is valid only while you have active Haigekassa coverage.

Pharmacies (Apotek)

Hours:Most apoteks: Monday–Friday 09:00–18:00 (some until 20:00); Saturday 09:00–15:00; Sunday closed (many) or shorter hours. Tallinn centre: some pharmacies open until 21:00. 24-hour pharmacies: Ülemiste Apotek (Ülemiste Centre, open 24/7), Viru Apotek (Tallinn centre, late hours).
