Healthcare in Portugal

Portugal's Serviço Nacional de Saúde (SNS) is a universal public healthcare system funded through general taxation, established by the 1979 Constituição da República. Coverage is universal for legal residents and extended to undocumented immigrants for primary care and emergencies. The SNS provides primary care through Centros de Saúde (health centres) with assigned médicos de família (GPs/family doctors), specialist care through hospital outpatient departments (consultas externas), and emergency services (urgência) 24/7 at public hospitals. Quality is generally good, particularly in the major urban centres (Lisbon, Porto, Coimbra), with excellent outcomes in oncology, cardiac surgery, and maternal care. Main weakness: long waiting lists for non-urgent specialists (3–18 months), limited availability of médicos de família in some areas, and pressure from the housing boom increasing demand in Lisbon/Porto.

Ranked 12th in the 2000 WHO ranking. Portugal ranks in the top 20 globally for healthcare outcomes on most OECD indicators. Life expectancy 82+ years. The SNS consistently delivers high-quality care for those registered with a médico de família. Private sector (CUF, Luz Saúde) is excellent and used widely for speed and specialist access.

National Health Service

Eligibility:Universal for Portuguese citizens and legal residents. All EU/EEA citizens with EHIC (Cartão Europeu de Seguro de Doença) have the right to necessary healthcare during stays. Legal residents (with valid residence permit or Certificado de Registo) are entitled to register with the SNS. Non-EU residents: register after obtaining residence permit. AIMA delays mean some legal residents wait months before receiving their permit — in this period, use private insurance.

Monthly cost:Free at point of use for most services (GP consultations now free; hospital emergency taxa moderadora of €14.30 applies). No monthly premium. Funded via general taxation and Segurança Social contributions.

Co-payments:GP consultations at Centro de Saúde are free (taxa moderadora abolished for primary care). Hospital outpatient specialist (consulta externa): €8.25. Hospital emergency (urgência): €14.30 (capped at €40 total per episode). Exemptions from remaining taxas moderadoras: children under 18, pregnant women, pensioners below income threshold, beneficiaries of RSI (minimum social income), military, blood donors, voluntary firemen, and chronic disease patients. Prescriptions: co-payment 15–90% of cost depending on medication category and income.

How to enrol:1. Obtain NIF. 2. Register with AIMA as a resident (or have EU Certificado de Registo). 3. Present NIF + residence document + proof of address at any Unidade de Saúde Familiar (USF) or Centro de Saúde. 4. Receive Número de Utente SNS (health service user number). 5. A médico de família may be assigned — or you may be on a waiting list if local GPs are at capacity (common in Lisbon and coastal Algarve).

Waiting period:SNS registration can be done soon after arrival. Médico de família assignment may take weeks to months in Lisbon/Porto/Algarve. Non-urgent specialist waiting times: 4–18 months. Urgent referrals faster. Emergency: immediate.

Private healthcare: Seguro de Saúde Privado / Planos de Saúde

Who can use it:Open to all. Wide range from basic plans (€30–60/month for young adults) to comprehensive international plans (€200–500+/month). Many Portuguese workers receive private health insurance as an employment benefit.

When it makes sense:Strongly recommended for: (1) New arrivals before SNS registration or during AIMA processing delays. (2) Those who cannot get a médico de família assigned immediately. (3) Families with children needing paediatric care without long waits. (4) Anyone wanting faster specialist access (private appointments in 1–2 days vs 6–12 months in SNS). (5) Dental care (almost nothing covered on SNS for adults). Typical annual cost vs the value of faster care makes private insurance excellent value in Portugal.

Monthly cost:€30–70/month (young adult, basic plan), €70–130/month (mid-range, 30–50 age), €130–300+/month (over 60 or with pre-existing conditions). Family plans: €100–250/month for family of 4.

Accessing care

GP system

If assigned a médico de família at your local Centro de Saúde/USF: book appointments by phone or online (MySNS app). Some USFs accept walk-in for urgent matters. If you are on a waiting list for a GP, you can use "Utentes Sem Médico de Família" services — urgent care at any SNS unit without an assigned GP. SNS24 nurse triage line (808 24 24 24, free) advises on appropriate care level.

Specialist referral

Referrals from médico de família to hospital consultas externas (outpatient specialists) go through the sistema de marcação centralizada. Waiting times vary: 3–6 months (routine), 1–4 months (urgent), immediate (very urgent). Private insurance bypasses all waits — same/next day specialist appointments in CUF, Luz, etc.

Emergency care

112 for life-threatening emergencies. Hospital Urgência (A&E) for urgent non-life-threatening: walk in, triaged by Manchester Triage System (colour-coded). No referral needed. INEM (Instituto Nacional de Emergência Médica) operates ambulances. Average wait: 1–4 hours depending on severity. Taxa moderadora €14.30.

Out of hours

SNS24 (808 24 24 24) — nurse triage service, 24h. Hospital Urgências are open 24/7. Some Centros de Saúde have extended hours or weekend services — check your ACES (Agrupamento de Centros de Saúde) website.

Prescriptions

Receitas electrónicas (electronic prescriptions) issued by your médico de família, valid at any farmácia. Co-payment: 15% (essential medicines), 37% (other included), 55% (intermediate), 90% (included for symptomatic treatment). Chronic medication prescriptions valid for 6 months with multiple dispensations.

Dental

Public dental care very limited for adults — mainly emergency extractions. "Cheque-Dentista" programme: free annual dental check for pregnant women, children up to 16, seniors 65+. Adults pay privately: check-up €50–80, cleaning €60–100, filling €60–120, implant €1,500–2,500. Many private dental clinics have competitive pricing; dental tourism from UK/France is significant. Budget option: dental schools at Faculdade de Medicina Dentária (Porto/Lisbon) offer supervised treatment at 50% discount.

Mental health

SNS mental health services: Equipas de Saúde Mental comunitária attached to hospitals, accessed via GP referral. Waiting times: 3–12 months for psychologist; psychiatry faster for acute cases. Private psychologists: €50–90/session. Online therapy platforms (Psicologia.pt marketplace, Talkspace, BetterHelp in English). SOS Voz Amiga and Linha do Apoio Psicológico for crisis support.

Maternity

Excellent free prenatal care through SNS — monthly consultations with médico de família and obstetra, free ultrasounds, blood tests, and childbirth. Most births in SNS hospitals are free. Epidural available. Postnatal home visits by enfermeira especialista (specialist nurse). Private maternity: CUF, Luz Saúde, and Lusíadas offer premium private birth suites (€3,000–6,000 extra for private room and care level).

Finding English-speaking doctors:CUF Hospitals and Luz Saúde clinics in Lisbon and Porto have English-speaking doctors throughout. Hospital da Luz Lisboa, Hospital CUF Descobertas (Lisbon), Hospital CUF Porto, Hospital da Luz Porto, and Hospital de Cascais all widely offer English-language consultations. In the Algarve: Hospital Lusíadas Albufeira, HPA Health Group (Portimão, Alvor, Gambelas/Faro) have excellent English services. SNS hospitals in Lisbon/Porto have some English-speaking staff; language support via SNS interpreting service.

International insurance:D-series visa holders (D7, D8, D2, D3) must hold private health insurance at least until SNS registration is complete. After SNS registration, many expats maintain private insurance for speed and dental. International plans (Cigna Global, Allianz Care, Aetna International) work in Portugal and are especially useful for frequent travellers.

Emergency card info:Carry your Cartão de Utente SNS (physical or in MySNS Wallet app) + NIF card + Cartão de Cidadão or passport. 112 for emergencies. Know your nearest hospital Urgência — note it in your phone. For English-language emergency info: CUF and Luz Saúde hospitals have English-language emergency lines.

EHIC notes:EU/EEA citizens: EHIC (Cartão Europeu de Seguro de Doença) covers necessary treatment during visits to Portugal, not as a replacement for SNS registration once resident. After establishing legal residence, EU citizens must register with SNS directly.

Pharmacies (Farmácia)

Hours:Typically Mon–Fri 9:00–19:00/20:00, Sat 9:00–13:00. Rotating 24-hour farmácia de serviço (on-call pharmacy) system: find the nearest via farmaciasdeguardia.pt, the green cross sign outside pharmacies, or by calling any pharmacy.

After-hours / emergency:Farmácia de serviço: rotating 24-hour on-call service. Posted outside every pharmacy and on farmaciasdeguardia.pt. Night surcharge (sobretaxa nocturna) typically €0.50–1.50 for after-hours dispensing.

Prescription rules:Receitas electrónicas: presented via MySNS app QR code or paper prescription. Farmacêuticos (pharmacists) can provide advice on OTC medications and some minor treatments without prescription. Antibiotics and most prescription-only medications (MSRM) require a doctor's prescription. Some medications available OTC in Portugal require prescription elsewhere (e.g. oral contraceptives, some antifungals).
